INSURANCE & RATESInsurance The Ultimate Choice Consulting, LLC is contracted with several insurance companies and Employee Assistance Programs (EAP) to provide counseling services. Check your insurance plan or employee benefits for coverage of mental health, it is the client responsibility for copays and deductible - coverage does not guarantee payment. Self-pay clients Sliding scale fees are available for self-pay clients who do not have insurance or employee benefits. Proof of income is required to assess the discounted rate for services. All clients are required to pay the initial session fee of $150 per individual and $175 per couple/family(excluding Employee Assistance Programs). A $50 deposit is required to schedule your first appointment. This fee is required at the time of service and is not billed with the insurance. Rates Initial Session $150/$175 Individual Session $200 Couple/family Session $250 No show/Late Cancellation $75 Payment is expected at time of service. If payment is not made your next session may be canceled. Debit/Credit/Flex Health cards are accepted. All payments are made on SimplePractice client portal.

** Fees differ markedly when contract rates with insurance plans apply. You would only be required to pay the difference between the contract rate and what your insurance plan covers. In most cases where insurance is being processed, this might be just a co-pay. However, if the deductible has not been met then the client is responsible for the allowable contract rate. Having insurance does not guarantee payment for services. If the deductible is met then your co-pay would be your fee. If the deductible is not met then the entire allowable is your fee.
